Insurances

As a courtesy to our patients and to avoid any financial misunderstandings, especially after care is rendered, we will contact your insurance company to verify your insurance benefits before your new patient visit. So that we can verify your insurance benefits, please be sure to have the following information available when calling Franco & Co:

  • Patient’s legal name and date of birth
  • Name of the primary insured
  • Insurance card of the primary insured (active)
  • Social security number of the primary insured

Please call us at: 954.436.7400 to provide us with your insurance related information so that we can verify your insurance benefits. It usually takes us 48 hours to verify your insurance benefits. Once your insurance benefits are verified and reviewed with you, we will schedule your new patient appointment. When calling us, please be sure to let us know if you would like your new patient visit scheduled at our CityPlace Doral or Miramar Town Center office.

New Patients: before you make a new patient appointment online, please visit our list of accepted insurances to see if we accept your insurance. If your insurance is not listed on our list of accepted insurances, please call our office for further guidance.

The day of your first visit, please make sure to bring: (1) active insurance card and (2) valid US government issued ID. We collect co-payments, co-insurances, and deductibles before you see the Doctor. For your convenience, we accept cash and all major credit cards. We do not accept checks.

  • If at any time before or after an appointment with us you believe that you are experiencing a medical emergency, please call 911 or go to your local emergency room. If you are admitted to an emergency department, Urgent Care facility, or any other facility it is your financial responsibility to arrange for payment.
  • Arrive on time to your appointment. We reserve the right to cancel or reschedule your appointment for late arrivals.
  • If you are assisted by a walker, cane, or other device, please let us know before your arrival.
  • Although it is nice to have visitors come with you to your appointment, our waiting room and treatment rooms do not accommodate additional people. Unless you are a minor, require assistance during your appointment, and or are scheduled for a surgery, we kindly request that you come unaccompanied.
  • If the appointment is for a child or someone requiring assistance, an adult must be physically available during the appointment.
  • If the appointment is for a surgical procedure, a person who can monitor and drive you must accompany you.
  • Bring a valid ID and active insurance card. Without a valid ID and active insurance card you will not be seen unless you are planning to pay as a self-pay (e.g. cash) patient.
  • It’s your responsibility to let us know when your insurance has changed as you will be billed for nonactive or incorrect insurance member ID numbers provided.
  • Bring with you a current list of medications.
  • Bring with you the full name and contact information of your primary care physician or referring physician.
  • Bring with you your pharmacy telephone number and address.
  • Know your patient financial responsibility. Copayments, co-insurances, and deductibles will be collected at check-in. We accept all major credit cards, CareCredit and cash. No checks will be accepted.
  • If you wear nail polish, please make sure that all nail polish is removed even though only one toe requires evaluation on one foot.
  • If you have current medical records from another healthcares provider (e.g., X-rays, imaging/diagnostic reports, blood work reports, clinical progress notes) please bring them with you.
  • If we gave you an order for a diagnostic procedure such as an MRI, CT scan, NCV, etc. please make sure that the diagnostic center has faxed us your report well before your appointment.
  • When getting a diagnostic procedure request a CD with your images. Please note that the diagnostic center may charge you for the CD with images.
  • No food or beverages are permitted in our offices except water in a no shatter container.
  • Plan to make your personal calls before or after your appointment. Private conversations should be kept private, however, should you need to take or make an urgent call, there are plenty of common areas outside of the office.
  • Please let us know before your arrival if you are seeking a second medical opinion.
  • Please let us know before your arrival if your appointment is related to a workers compensation claim. We do not process workers compensation claims.
